Direct Primary Care (DPC) Membership Terms & Conditions
Effective Date: February 19, 2026
By enrolling in a Direct Primary Care (“DPC”) membership with Compass Family Practice, you acknowledge and agree to the following terms and conditions:
1. Nature of Direct Primary Care Membership
Direct Primary Care is a membership-based arrangement for primary care services and is not health insurance.
This membership does not satisfy any federal or state requirement to maintain health insurance coverage.
Members are strongly encouraged to maintain separate health insurance coverage for services not provided under this agreement.
The Member is financially responsible for all healthcare services not expressly included in the DPC membership, including but not limited to:
- Hospitalization
- Emergency department care
- Specialty care
- Advanced imaging
- Surgical procedures
- Any other services not provided directly by Compass Family Practice
2. Membership Fees and Billing
Membership fees are due in advance according to the selected billing cycle (monthly, quarterly, or annually).
Monthly or Quarterly Memberships
Membership fees are non-refundable for the current billing period.
No partial refunds will be issued for unused time within an active billing cycle.
Annual Memberships (Paid in Full)
Prorated refunds may be issued only for unused, full calendar months remaining in the membership term.
All prorated refunds are subject to deduction of the $75 enrollment fee.
No refunds will be provided for partial months.
3. Cancellation by Member
Members may cancel their membership at any time by providing written notice to Compass Family Practice.
Cancellation becomes effective at the end of the current billing period unless otherwise agreed in writing.
The Member remains financially responsible for all fees due through the effective date of cancellation.
No refunds will be issued except as described above for annual memberships.
4. Re-Enrollment Policy
Following cancellation, a Member must wait one (1) year before re-enrolling.
Early re-enrollment within one (1) year of cancellation requires payment of all unpaid membership fees that would have been due during the lapsed period before services may resume.
Compass Family Practice reserves the right to approve or deny re-enrollment at its discretion.
5. Termination by Practice
Compass Family Practice reserves the right to terminate the patient–provider relationship at its discretion, in accordance with applicable state and federal laws, upon reasonable written notice.
Grounds for termination may include, but are not limited to:
- Nonpayment of membership fees
- Repeated missed or late appointments
- Disruptive, abusive, or inappropriate behavior toward staff or providers
- Failure to comply with clinic policies
- Situations in which the provider determines that the therapeutic relationship is no longer effective, safe, or appropriate
In the event of termination:
- Compass Family Practice will provide urgent or emergency care for a reasonable transition period, as required by law.
- The practice will assist with the transfer of medical records upon receipt of a written authorization.
6. No Insurance Billing
Compass Family Practice does not bill insurance for services included under the DPC membership.
Members acknowledge that they may not submit DPC membership fees for reimbursement from insurance plans unless specifically allowed by their insurer.
7. Modification of Terms
Compass Family Practice reserves the right to update or modify these Terms and Conditions.
Members will be provided reasonable notice of any material changes.
